Exclusive Acuvue Rebates for VSP Members

Beginning October 1, 2010, VSP (Vision Service Plan)  members who purchase a qualifying supply of select ACUVUE Brand Contact Lenses from a VSP doctor within 90 days from their exam are eligible to receive an extra $25 on top of the current national rebate program.

Currently, rebates are available on the following contact lens brands:

• 1-Day ACUVUE^® TruEye™
• 1-Day ACUVUE^® Moist^®
• ACUVUE^® OASYS^® with HYDRACLEAR^® Plus
• ACUVUE^® OASYS^® for ASTIGMATISM
• ACUVUE^® OASYS^® for PRESBYOPIA

Who is eligible?

All VSP Members, regardless of plan coverage, are eligible for the additional $25 rebate when they purchase a qualifying supply of select ACUVUE Brand Contact Lenses from a VSP network doctor within 90 days of their exam. Please note: CIGNA Vision and Medicaid members are not eligible for this rebate.

How does it work?

It’s simple—all you have to do is follow these three easy steps:

1. Get an exam from a VSP doctor.
2. Purchase the required number of eligible ACUVUE lenses from a VSP doctor within 90 days of their eye exam.
3. Complete and mail in the VSP and ACUVUE Contact Lens Rebate Form. When submitting their rebate you will need the following:
   • Completed VSP and ACUVUE Contact Lens Rebate Form
   • Two eligible box tops
   • Eye exam receipt
   • Product purchase receipt(s)
